บทคัดย่อ

การวิจัยและพัฒนา (The Research and Development) ครั้งนี้ มีวัตถุประสงค์เพื่อ 1) ศึกษาการดาเนินการสนับสนุนการดูแลและติดตามภาวะสุขภาพของผู้สูงอายุที่เป็นผู้ป่วยเบาหวานของชุมชน 2) พัฒนาระบบสนับสนุนการดูแล และติดตามภาวะสุขภาพสาหรับผู้สูงอายุที่เป็นผู้ป่วยเบาหวานในชุมชนโดยประยุกต์ใช้ระบบสารสนเทศทางภูมิศาสตร์ กลุ่มตัวอย่าง จานวน 210 คน คือ ผู้สูงอายุที่เป็นผู้ป่วยเบาหวาน 160 คน และผู้ดูแลผู้สูงอายุที่เป็นผู้ป่วยเบาหวาน เจ้าหน้าที่สาธารณสุข และอาสาสมัครสาธารณสุขประจาหมู่บ้าน (อสม.) จานวน 30 คน ดาเนินการวิจัยในพื้นที่ตาบลปางหมู อาเภอเมือง จังหวัดแม่ฮ่องสอน ใช้วิธีการสุ่มตัวอย่างเป็นระบบ เครื่องมือที่ใช้รวบรวมข้อมูล คือ แบบสอบถาม วิเคราะห์ข้อมูลด้วยความถี่ ค่าเฉลี่ย ส่วนเบี่ยงเบนมาตรฐาน และการวิเคราะห์รูปแบบการกระจายตัว ค่าดัชนีเพื่อนบ้านใกล้ที่สุด (Average Nearest Neighbor) 
ผลการศึกษา มีดังนี้ 1) การสนับสนุนการดูแลและติดตามภาวะสุขภาพของผู้สูงอายุที่เป็นผู้ป่วยเบาหวานดาเนินการแบบบูรณาการระหว่างเจ้าหน้าที่สาธารณสุข ผู้ดูแล ครอบครัว เครือข่าย อสม. ในชุมชนสนับสนุนส่งเสริมการปรับเปลี่ยนพฤติกรรม การไปเยี่ยมบ้าน และการดูแลในระยะยาว โดยที่ อสม. มีบทบาทสาคัญต่อการกระตุ้นเตือนให้ดูแลเท้า ลดอาหารหวานมันเค็ม การออกกาลังกาย การไปพบแพทย์ตามนัดหมาย 2) ได้แผนที่แสดงพิกัดและเส้นทางเข้าถึงที่พักอาศัยของผู้สูงอายุที่เป็นเบาหวาน ผลการวิเคราะห์ค่าดัชนีเพื่อนบ้านใกล้ที่สุด สรุปว่า มีการกระจายตัวแบบเกาะกลุ่มใกล้กับ รพ.สต. และส่วนใหญ่มีระดับน้าตาลในเลือดปกติ การพัฒนา Web Application ฐานข้อมูลผู้สูงอายุที่เป็นผู้ป่วยเบาหวาน สามารถแปลผลค่าน้าตาลในเลือด ค้นหาข้อมูลและแสดงผลได้อย่างเฉพาะเจาะจง จึงมีส่วนช่วยเพิ่มประสิทธิภาพการเข้าถึงข้อมูลและติดตามดูแลเป็นรายบุคคลและภาพรวมเชิงพื้นที่

Abstract

This research aimed to 1) study the community process of care and monitoring elderly diabetic patients. 2) apply the Geographic Information System (GIS) in developing the community care and monitoring support system for elderly diabetic patients. The study employed the research and development (R&D) method, which involved an iterative process of designing and testing the system, incorporating feedback from stakeholders, and refining the system based on the results. The study was conducted in Pang Moo Subdistrict, Mueang Mae Hong Son District, Mae Hong Son Province. 210 samples were selected using the Systematic Random Sampling method, which included 160 elderly diabetic patients and 30 people from patients' caregivers, healthcare workers, and village health volunteers. Data were collected using questionnaires and were analyzed using frequency, means, standard deviation, and average nearest neighbor (spatial statistics). 
The study results revealed the following: 1) The process of care and monitoring involved integration of caregivers, healthcare workers, and village health volunteers, focusing on enhancing behavioral change, home visit programs, and long-term care. Village health volunteers played a crucial role in promoting DM foot care, healthy eating, exercise, and regular doctor visits. 2) The spot map created on GIS software showed geographic coordinates and routes to individual elderly diabetic patients' residences. The statistical analysis revealed the significant clustering of the patients in each Tambon Health Promoting Hospital area (using the average nearest neighbor index), and most patients had normal fasting blood sugar levels. The patients' database web application was created to support care and monitoring works by providing a user-friendly responsive web interface with the ability to search, sort, visualize blood sugar levels in four different colors, and open a google maps location of individual patient residences, resulting in more effective care and follow-up for elderly diabetic patients.
